Open Enrollment for group health, vision, and dental plans began Tuesday, April 2, 2013. The final date to change your current plan selection and/or add or delete spouses/dependents is Tuesday, April 16, 2013.
Human Resources will host TWO OPEN ENROLLMENT BENEFITS FAIRS; Monday, April 8, 2013 in Alumni Hall from 3:00 p.m. to 5:00 p.m. and Thursday, April 11, 2013 in Main Dining Room from 11:00 a.m. to 2:00 p.m.
Representatives from Blue Cross, Total HMO/COFINITY, Ameraplan, Value Options (Employee Assistance Provider), Delta Dental, DenCap, and Mutual of Omaha will be presenting (see times listed below) and available to respond to your inquiries.
Remember this is the only time that you will be able to change your coverage, add/drop dependents/spouses unless there is a qualifying event (marriage, divorce, birth, adoption, lay-off or loss of coverage).
Employees, who choose not to make any changes, must complete the Enrollment and Account Election Form and return this page only by Tuesday, April 16, 2013. Those making changes must complete the entire Enrollment and Account Election Form by the deadline.
Annual part-time employees may participate in the group health, vision and dental plans by paying the full premium cost.
